Smile Makeover Information We are currently taking applications/nominations for our next makeover, which we hope to complete in November of 2010. Please print off the application form here and return the completed form to our office address: Dr. Brown and Dr. Copeland's Dental Office ATTN: Smile Makeover 912 Osler Drive, Suite A Jonesboro, AR 72401. The applicant will need to be a current or new patient of our office. This means that if you are not an existing patient, you will need to come in for a full check up to see if you are a candidate for the Smile Makeover. Patients who have not been to a dental office for many years, or who have moderate to severe periodontal disease may not qualify for the makeover until their mouth is healthy and healed. That is why a full series of dental x-rays and check up is vital. Once you have been through the new patient exam, we will be happy to take your application. Existing patients who want to apply will need to have had a cleaning and check up within the year before the selection date If you choose to begin the application process and set up your new patient appointment, please feel free to call our office or email me and we will locate an appointment that is convenient to your schedule.
